This program has been approved for AMA, PRA Category 1 CME through the joint sponsorship of Seton Hall University School of Graduate Medical Education and the American Academy of Continuing Medical Education, Inc. Seton Hall University School of CME is an accredited provider of Continuing Medical Education. Continuing Education credit will also be provided for nurses and pharmacists.

In order for us to send you the materials necessary for participants attending this program to receive CE credit, we request that you agree to the following conditions. Once we have received this signed agreement, we will forward a sign-in form, an evaluation form, and certificates of attendance.

• Have each attendee sign-in on the sign-in form that we will provide.
• Have each attendee receive and complete our evaluation form that you will receive in advance.
• Complete an on-site moderator evaluation form.
• Collect the completed evaluation forms at the conclusion of the program.
• Distribute certificate of attendance to only those attendees that return a completed evaluation form.
• Return to AACME, within 10 days, the sign-in sheets, completed evaluation forms, and any left over certificates.
