Note: This registration form is for
hospitals and other institutions wishing to register at no cost
for the broadcasts. Please read below and "submit" to
receive the registration materials by e-mail.
This activity is sponsored by the Albert Einstein College of Medicine and Montefiore Medical Center.
 
In order for
us to send you the materials necessary for participants attending
this activity to receive CE credit, we request that you agree to
the following conditions. Once we have received this signed agreement,
we will forward a sign-in form, an evaluation form, certificates
of attendance, and moderator information form.
I agree to:
- Have each attendee sign-in
on an individual sign-in form that is provided.
- Have each attendee receive
and complete our evaluation form that you will receive in advance.
- Complete an on-site monitor's
evaluation form.
- Collect the completed evaluation
forms at the conclusion of the program.
- Return to us, within 10 days,
the sign-in sheets and completed evaluation forms. The participants
will receive certificates directly from Albert Einstein College
of Medicine and Montefiore Medical Center.
